On the assumption that this is more than a local problem... System: Sun 4/300 running SunOS 4.1.3_U1 with serial console on ttya. System is original GENERIC kernel - nothing modloaded, clean as a whistle. Put a serial terminal on ttyd and enable it. Method 1: Login as root on ttyd. *THUD* Method 2: Type anything on ttyd. Have you installed patch 101621-02. This is the SunOS 4.1.3_U1 Jumbo tty patch. We were having a similar problem with a 4/330 with a laser printer attached to /dev/ttyd. Installing this patch has completely fixed the problem. BTW, this is one of Sun's "recommended" patches. I would suggest installing these patches as there are some security related patches here.
