I really can't comment on installing 2003 with ISA at all, but I would say that in the past I have always done this with a BSD box running Postfix and SpamAssasin, and things have worked really well. I have a test 2003 Srvr. with ISA on it and I have the 2003 SMTP service running - the 2003/ISA box will receive mail from the internet and the Ex Srvr will pull mail from ISA.
